New Patient Registration

Milton House Surgery is now open for registrations from new patients.

Patients must be permanent residents of the UK and live within our practice catchment area (click here to check your postcode).

New registrants will need to provide photographic proof of ID and a recent utility bill or current Council Tax statement as proof of address. For children, their parent or guardian will need to provide the appropriate ID where necessary.

We are required to have both an inner and an outer practice boundary.

A Map of the Practice Area can be found here.

The inner boundary is denoted by the solid black line and any person residing within this area for a minimum of 6 months may register with this practice.

The outer boundary is denoted by a red dashed line. Any person who was residing within our inner area may remain registered with this practice if they move somewhere within the outer boundary.

HOWEVER, anyone not previously registered at this practice who lives in the outer area i.e. between the solid black and the dotted lines will not be able to register at this practice.
